Families Together Achieves Patient-Centered Medical Home (PCMH) Status, Enhancing Care for Orange County Residents

TUSTIN, Calif., March 28, 2024 – Families Together of Orange County Community Health Center (FTOC), a nonprofit Federally Qualified Community Health Center Look-Alike, is proud to announce that it has achieved recognition as a Patient-Centered Medical Home (PCMH). This prestigious designation, granted by the National Committee for Quality Assurance (NCQA), signifies Families Together’s commitment to providing high-quality, patient-centered care to its community.

PCMH status is awarded to healthcare organizations that meet rigorous standards for care coordination, patient engagement, and continuous quality improvement. By obtaining this status, Families Together has demonstrated its dedication to delivering comprehensive, coordinated care that addresses the needs and preferences of each individual patient.
