PARA PUBLICACIÓN INMEDIATA

TUSTIN, Calif., August 22, 2025 – Families Together of Orange County (FTOC) has been honored by the U.S. Health Resources and Services Administration (HRSA) as a 2025 National Quality Leader, one of the highest recognitions awarded to community health centers nationwide.

In addition to earning the National Quality Leader Badge for Diabetes, FTOC also received the Preventive Care Badge and the Improving Health Care Access Badge — distinctions that reflect the organization’s exceptional performance in delivering quality care and expanding access to vital services.

Over the past year, FTOC has placed a strong focus on improving quality across its programs, and that commitment has paid off. The organization worked diligently to ensure its integrated care teams collaborated seamlessly so that patients at risk for diabetes, or already living with the disease, received comprehensive, coordinated care. From vision screenings to podiatry services and other essential preventive measures, FTOC ensured that every patient had access to the resources needed to manage and improve their health.

  • National Quality Leader Badge (Diabetes) – Awarded to health centers achieving top-tier clinical outcomes in diabetes care, placing FTOC among the best-performing centers in the nation.
  • Preventive Care Badge – Recognizes FTOC’s commitment to comprehensive preventive services that support early detection and intervention.
  • Improving Health Care Access Badge – Highlights FTOC’s success in reducing barriers to care and ensuring more patients receive the services they need.
